In Community Habilitation our goal is to assist participants to reach their individualized potential and develop relationships within their local community.

Community Habilitation supports participants, in a one-on-one setting, to develop new skills and practice these new skills by utilizing community resources as well as their own homes to work on skill building. We value supporting our participants to reach their full potential, growth and independent skill development.

Community Habilitation plans are developed, with the participant, family and staff in those areas that need to be strengthened in order to develop their independence skills.  Our staff members work with their participants on improving skills in the areas of:

  • Personal Hygiene

  • Money

  • Prosocial interactions

  • Housekeeping

  • Self-advocating

  • Life-safety

  • Travel training 

  • Self-care

We currently serve participants in Madison County. Our program provides resources for costs of activities that are related to the individual’s goals, meals for the staff and participant during service hours while practicing remaining within their monthly budget.  Community Habilitation can also assist participants to a set number of physician or clinic visits annually. 

The focus of Community Habilitation is to familiarize the participant with their local community. The purpose is to develop strong relationships with the stores, agencies, services and neighbors in order to be more self-sufficient.

The Community Habilitation manager matches up staff with areas of skill strength that the participant needs support with. Other areas to consider when matching up staff-participant pairs, is the distance between their two homes, their availability for services and the amount of hours the two can fit into their schedule.
