ADMISSIONS PROCEDURE
1. At the time of initial contact, the Navigation Coordinator Office will provide a copy of the Program Overview that includes a full program description and the initial application. Also included in that packet will be information about admission criteria, all other Heritage Farm program overviews, available transportation options and admission requirements.
2. Once the pre-screening application is completed and signed, it will be submitted to the Navigation Coordinator Office for review. The Navigation Coordinator Office will review the application within 5 (five) business days.
3. If the Navigation Coordinator Office identifies potential problems regarding the initial application, it will be distributed to the Admissions Committee for further review. Upon committee review, a determination will be made regarding whether or not the application process should continue. If it is determined that the process should end, the Navigation Coordinator Office will notify the Service Coordinator or other appropriate person(s), otherwise, the process continues.
4. Upon review, if the initial application is complete and an opening exists or is anticipated, the NavigationCoordinator will schedule a facilities tour.
5. Upon completion of the tour, the family may decide whether or not they want to pursue formal admission. If they choose not to, the process ends at this point. Should they choose to continue the process; the Navigation Coordinator Office will provide the full application for services, along with a clear written list of all required assessments, paperwork, previous plans, etc. The Navigation Coordinator Office will act as a resource to the individual and their family, Care Manager or advocates when completing the application and gathering all required information.
6. Upon receipt of the completed full application and all accompanying documentation, the admissions committee will convene within 5 (five) business days to review the application. An incomplete application will not be eligible for further review, and the process will stop here until such time as all required documentation has been received. Upon review of the completed application, if the admissions committee determines that the individual does not meet admissions criteria, the Care Manager or other appropriate person(s) will be notified. If the committee determines that the application process should continue, the Navigation Coordinator Office will schedule a minimum of 3(three) full day visits.
7. Once the day visits have been scheduled, the Navigation Coordinator Office will distribute the appropriate information to all involved staff members prior to the first visit.
8. As each day visit occurs, the Navigation Coordinator Office, in conjunction with the Day Habilitation Manager or other appropriate staff member will complete the Initial Assessment Form. A minimum of 3(three) day visits will take place. After the third visit, the Admissions Committee will meet within 5(five) business days to review the documentation associated with the visits. During this meeting, one of 3 (three) options will be selected:
a. The decision to admit will be made, the initial Day Habilitation Plan will be developed, transportation will be arranged, and a formal admission letter will be sent to participant, Care Manager and/ or advocates.
b. A decision to continue the evaluation period with another set of 3 (three) additional day visits may be made.
c. Admission will be denied, with a full written explanation provided to the family by the Navigation Coordinator Office.
